📘 Requirements for Freshmen Students
- Original and Photocopy of the High School Card / Senior High School Card (F-138a)
- Certificate of Good Moral Character (Original & Photocopy)
- Certificate of Live Birth (PSA Copy)
- Certificate of Marriage (PSA Copy, for married women)
- Medical Certificate (for Medical courses only)
- Certificate of Honors (if applicable)

- Certificate of Transfer Credential
- Certificate of Good Moral Character
- Marriage Contract (for married women)
- Birth Certificate (PSA Original)
- 2×2 pictures with name tag (white background, 4 pcs)
- 1 long folder
📘 Requirements for Graduate School
- Transfer Credential (Honorable Dismissal)
- Transcript of Records (Photocopy for Evaluation)
- Birth Certificate (PSA Photocopy)
- 2×2 pictures with name tag (white background, 4 pcs)
- Marriage Contract (for married women)
- 3 long brown folders
Registration Guidelines
Follow these step-by-step instructions to successfully complete your registration and admission process.
📝 Registration in a Subject
Any student who enrolls and pays the corresponding fees shall attend enrolled subjects from start to end of the term. Failure to do so results in a grade of 9.0 (dropped), unless officially dropped through the Student Accounts Office.
📚 Maximum Subject Load
The subject load of students is based on the approved curriculum of each program of study.
🔄 Cross Enrollment
Cross-enrollment is allowed when desired subjects are not offered in the current term or during summer in the student’s home region. Written permission from the Dean is required before cross-enrollment can be credited.
