  4. Original and Photocopy of Certificate of Marriage (PSA) copy, for married woman
  5. Original Medical Certificate, for Medical courses only
  6. Photocopy of Certificate of Honors (if with honors)

  3. Photocopy of Marriage Contract (For Married: Female Only)
  4. Birth Certificate (PSA Original)
